PAYMENT: Your credit card holds your appointment. In lieu of a credit card I will accept Venmo. Sorry, but I don’t process or deal with insurance claims. 

Once we have determined a date and time for your appointment, I will email a confirmation message to you. Please click the button on that message and pay for the scheduled appointment with a credit card or send cash to my Venmo account.

APPOINTMENT REMINDERS: I will send an appointment reminder with the call information two days before your appointment. However, it is your responsibility to join the call on the agreed date and time. This gives you the benefit of the full session you’ve paid for and avoids awkward conversations and cancellation charges.

RESCHEDULING / CANCELLATION: Unavoidable things can happen in your life, that’s understandable. If you can’t make your appointment and need to change it, please call me at 425-299-0885.

If you are a no-show to your appointment or don’t cancel or request a reschedule 24 hours before your appointment, you will be charged as noted below.

RESCHEDULING: To reschedule, please call at least 24 hours before the scheduled appointment.

CANCELLATION: If you need to cancel, please contact me immediately. This way someone else can take that appointment. 
     . If you call fewer than 24 hours before the appointment, you will be refunded 50% of your session fee.
     . If you miss your appointment without calling or notifying me, none of your session fee will be refunded.

 

CONFIDENTAL HEALTH HISTORY: In order for me to provide you with the best care, I also need to be aware of any changes to your health or medications after I begin treating you. Your health and safety are my top priority. I ask that all patients complete a health history form prior to their first session (click the button below). This snapshot of your life story provides valuable information about your health and ensures that I can deliver personalized services tailored to meet your specific needs.
